          Junior Instructors Application 2019
Completed applications must be dropped off at the Jefferson Township Police Department, located at 1033 Weldon Road, by 5:00 p.m. on April 25, 2019. No late applications or extensions will be granted or accepted.

          Vacant Home Form & Security Checklist
1. If you complete this form and the email link at the bottom is not functional, try opening www.jeffersonpolice.com in Internet Explorer, then open the form again. This should resolve the problem. If not, use the following work around: (a) complete then save the vacant home form as a PDF on your computer; (b) Open your email program and prepare an email to squenault@jeffersonpolice.com. (c) Click the "attach" button in your email program and navigate to the vacant home form you saved as a PDF and click on it to attach it to the email; and (d) hit "Send." 2. If faxing the document, PLEASE CALL 973-208- 6151 AFTER FAXING TO CONFIRM IT WAS RECEIVED AND IS LEGIBLE. We have received many vacant home forms that we cannot read and otherwise have no way of knowing who to contact for a clear copy, thus, if we cannot read the address, we won't be able to watch the house while you're away. 3.     Firearms Forms
          Firearms Application & Registration System (FARS)
The State of New Jersey is now accepting applications for Firearms Purchaser Identification Cards and Pistol Purchase Permits electronically through the Firearms Application & Registration System (FARS). FARS can be used for all firearms related applications except Permits to Carry. Carry Permits must still be applied for using the forms. While completing your application, you will be asked for the Jefferson Township Police ORI number, which is NJ0141400. IN ADDITION, during the FARS application process, you will be prompted to make an appointment with IdentiGO for fingerprints. After you have been fingerprinted, a PCN number will be assigned, and that number will be hand written on your IdentiGO universal form and/or will be on the receipt you are given. The PCN NUMBER AND DATE are necessary for JTPD to continue the application process, so you will need to provide the number to JTPD as soon after being fingerprinted as possible, in one of two ways: 1. You may personally bring this documentation to Janice Geib in the JTPD Records Bureau; or 2. You may scan and email the documentation to jgeib@jeffersonpolice.com.           Permit to Carry A Handgun Application
Please read this entire packet of forms and instructions before completing anything. There are important notes throughout that will help you complete it properly. Please also note that all paperwork submitted MUST BE SIGNED BY THE APPLICANT before being dropping off. PLEASE ATTACH A COPY OF YOUR DRIVER'S LICENSE AND FIREARMS ID CARD TO ANY APPLICATION YOU SUBMIT. Incomplete applications will not be processed. DO NOT ATTACH MONEY IN ANY FORM WHEN YOU DROP OFF YOUR APPLICATIONS. Thank you.

    Disabled Parking Placard Applications
          Permanent Disabled Parking Application
If you have a permanent disability, you must use this application to obtain a permanent (blue) disabled parking placard. Permanent Placards are issued by the NJ Motor Commission. With this application, you can obtain a Permanent Disabled Parking Placard and/or Wheelchair Symbol License Plates. You are permitted to have one of each if you wish. Make the appropriate selections on the application. When you have completed all sections that apply to you on this application, you must sign and date in all places required. You must then have your doctor complete his/her sections and sign and date on the appropriate line. A prescription IS required for a Permanent Placard or Plates. There is NO fee. If you require license plates, you must attach a copy of your current motor vehicle registration to the application. Bring your completed application, prescription, NJ Driver's License, and copy of your registration (if you are applying for wheelchair plates) to any local motor vehicle office. The placard (and corresponding disabled persons ID card) will be issued while you wait.
          Temporary Disability Placard Application
If you have a temporary disability from which you will recover in less than one (1) year, use this application to obtain a temporary disabled parking placard (red). You must fill this form out with your personal information and sign and date it, then take it to your doctor and have him/her complete and sign it. There is no prescription required for a temporary disabled parking placard, however, there IS a $4.00 fee. If you wish to wait while your placard is prepared, call 973-208- 6151 and make an appointment with Shelley. You may also drop off the application (and check or money order) off any time 24/7 and you will be contacted when it is ready. In this case, please write a phone number where you can be reached on the top of the application. All applications must be accompanied by a check or money order (NO CASH) in the amount of $4.00, payable to New Jersey Motor Vehicle Commission, or "NJ MVC."

          Police Services Application
If you require police services for crowd control, to manage parking, pedestrian traffic, traffic direction, etc., please print out and complete this form and submit the completed form, along with a Certificate of Insurance and an escrow check (see instructions on form). An officer cannot be scheduled for your job until you have turned in all paperwork and your check to Shelley in the Chief's Office as far in advance of your event as possible.

          Late Accident Report
If you are involved in a motor vehicle accident and do not report it right away, you must make your own report of the accident for insurance purposes or to report it to the State of New Jersey. To do so, fill out this form using the included guide, then print it out and manually make your drawing. Mailing instructions are right on the form.
